Enemies stop caring bug

I’ve encountered this bug 4 times in the last 2 days. Twice in a lost sector and more annoyingly in lawless and warlords ruin. It seems to be random when enemies just decided to stop working, for example in warlords first encounter the boss would drop the first hexes, I go through the cage and then after I come out the hexes can’t be captured, and the boss will never drop them again even after I waited over 10 minutes. I’ve noticed it has only occurred when I’ve played solo, I’ve updated my console and cleared the cache so I don’t understand what’s causing it.
