I'm convinced it's completely random and the truth is that most people just aren't very good. Not that I'm good either, but with the majority being at around my skill level, the odds of getting bad teammates is just waaaaaay higher.

It's completely unbalanced, whatever tf it is, rendering the entire ranking system and incentive to bother moot, and making suspensions for leaving rigged games a meme. There's no way my finishing a match with a15.0 kd and dying twice the whole game is the same level as my guy that died 24 times and got 1 kill. You know who is the same level? The entire opposing team, every single game.
