Sounds like you took to long to get his ghost. I get his ghost immediately and this hasn't ever happened to me. But then again I'm built different.

He died at base of stairs, it took the time to run from damage pool to ghost... Like 3 seconds

You do know you don't have to stay in that pool to damage him, right?
