Are you playing on last gen? It's common on my PS4 but I never see happen on recent consoles.
* I will be glad if after Final Shape Bungie cuts last gen support going forward. It's really holding the game back at this point.

Playing on series x, hasnt happened in awhile so I guess they fixed it

Playing on series x, and it doesnt happen all the time but it still occurs sometimes. Just a work around is to go to the scoreboard and give out commendations that way
