Hey there!
When did you get the triumph before that? It's called "Another Link in the Chain" and you need to complete it a week before attempting to get the "All the Scattered Pieces" triumph.
If you have completed the mentioned triumph this week, you will not be able to get the Ghost shell and the triumph.
