Only the first run per character class per week has the chance to drop new gear. Subsequent runs within the week as well as the secret chests will only drop items you've previously unlocked. It could just be coincidence if you're getting the same items in the same order, but it also depends on that encounter's loot table and what you've unlocked from it.
