It has to do with the scannables in presage. There are 3 triumphs there which gives you that triumph and a ghost shell when done. Each triumph is time gated per reset so it takes minimum, 3 weeks to unlock.

I tried doing scanables with a Fireteam bit we could not find anything.

check youtube for videos on the scanables they lock them so you can only scan certain ones each week
