Did you leave at any point during the run? Leaving the fireteam will result in losing the flawless triumph.

Luca, I did not leave at any point. If you look at the Raid.Report, one of my teammates swapped from warlock to titan, but he was the only one. After the boss was killed everyone else except me was given the triumph. Thanks

Thank you for the information. Was there anything unusual happening to you during the run? As in connection issues, enemies teleporting, joining allies, etc? As you might know, this triumph was previously bugged so you could not get it, but since then, it has been fixed (which is basically confirmed as your teammates got the triumph), so I'm thinking about potential issues that could have prevented you from getting the triumph. Also, do you happen to have a video or recording of this happening? This could help, but if you don't have one, it's okay.

Is there anyway to directly contact the help at Bungie? Or is posting in these forums and praying someone sees it really the only way?

The only odd thing would be that in the final encounter, I lagged out of Playstation party chat, my character was never returned to orbit and I was able to continue with the encounter mechanics as I normally would. I was able to re-join the party right before damage phase. Unfortunately I don’t have a video though.

Lagging out of Playstation party chat would indicate a connection issue. So, although you never left, it might have impacted your instance even though I am not sure about that.
