Just wanted to bump as I have the exact same issue. There was just 2 yellow bars that spawned and no triumph. There is also a bug with the Wisdom Of The Witch triumph giving no progress, but apparently Bungie is investigating this one. Finally I have not been able to get the time trial for the Keep of Honed Edges, despite making it in a very short time (I tried leaving the ascendant realm with and without the sword, I tried sword-flying as fast as I could with the hive sword, tried not sword-flying and just jumping from obstacle to obstacle, and tried it a dozen of times but still got nothing -trust me speed is not the issue-). I have yet to try it again the next time it's in rotation though, maybe it has been fixed because I got the Cimmerian Garrison time trial first try even though some people apparently have problems with that one on the forums.
