Yes, I had the exact same thing happen to me. I did it at the end of spending 16 hours in the raid on day 1 and thought I must have effed something up.
It consumed my buff and I couldn't pick up the scanner buff anymore during the week.

There appears to be a bug with this week's augment triumph as well. I have 2/3 for the Infinitude triumph, but somehow 1/3 for the Nexus triumph. Pretty sure one of the terminals is tagged with the wrong location.

Fwiw, I think last week I ended i up p shooting 2 of the reborn and then 1 of another set. I got the 3 for this week. I’m hoping I can cycle back to reborn when the set I accidentally shot the one from comes around. I just didn’t expect to be able to shoot one from the wrong set and be in a partially complete state on 2 of them.

People are destroying a set while they are still in the Nexus and then going into the Well and destroying two sets there. They could have destroyed all three Nexus sets by returning to the fork and finding the other two on the way to the Glassway. Or if they ignored the Nexus ones, reached the Well area, and carried on after the first two sets, they'd would have found the third and final set in the room where you fight the Brig in the Warrior empire hunt. That is what I did, and I am correctly showing as complete for the Well -> https://www.d2checklist.com/3/4611686018470750243/triumphs/tree/2322553724 Anyone who went 1/3 and 2/3 can just get the missing three another week when the Vex scanner is available. That'll complete both triumphs.

Yep it’s bugged the whole transponder is bugged. picked it up from variks and couldn’t pluck it from dead fallen said no way jose. so I have couldn’t complete this week. Last week was fine.

Ah, I see, that could be a little more obvious :/ Do you specifically have to destroy the set you missed (the one in the Brig room in my case) or can you destroy one of the other sets twice?

I do not believe destroyed ones respawn. I did not see any in Riis-Reborn Approach which is where I destroyed my first set last week. So yeah, you'd have to trek all the way to the Brig room to complete Augment: Well then backtrack all the way back to the Nexus fork and go left for the other two.
