Ace in the Hole Doesn't End
Successfully completed it once and went to do it again (for the 5 Forsaken heroic mission triumph) and the second run didn't end. I had to go to orbit and didn't get credit for my last weekly heroic mission or the triumph. Made a video and uploaded to YouTube for your viewing pleasure and analysis.
