I have this exact problem. I completed the event within the specified time frame, have a verified email and clicked, and confirmed, the button; but no T-shirt code is sent.

This is how it looks on the web page. Despite getting the mail sent-notice nothing is received, and yes, I have double and tripple checked my spam folder. I do get other Bungie mail for this account as well.
