Well....are you actually completing the challenges (located on right side of screen once you pull out your ghost while in a crucible match)? Can you screenshot what challenges are showing & their completion?

Yes they all show at 100% completed.....

I posted this from work I’m not playing now... by the time I get home later a new challenge will be in place... just making the comment as it’s something happening almost every week....

Well I personally havent had this happen nor have I seen reports of others having this happen. If it happens again, then it’s best to make a report with evidence so that the community/Bungie may be able to better assist you (and so Bungie can have proof in order to get the investigation going on their end).

So to best get a description of issue in order to have your 3 challenge crucible all others (ghost pull up) have to be completed?

Yes. Those (on right side of screen after pulling up ghost) are the challenges, which are needed for the daily milestone.
