You have to be very lucky to join back. Most likely you won't be able to. If the host got disconnected, you may be able to join during the host migration AND if it's betwen games as seen in my video. Quickly find your friend in the roster and join as soon as possible. You will get a loss for disconnecting but it will be nullified if you have Mercy of Osiris.
Plus, one of the team's score will be reset to zero. In my case, my team's score went to zero while the enemy was at 3. So we had to win 8 rounds. Which means we played an 11- round Trials Match.

That makes sense. What if one of our fireteam lost his internet connection and come back in 5 mins. Will that one count as a lost?

Yes. But if the game automatically reconnected him, he won't get a loss.
