Are you the one putting the Rune in the statue? If it's someone else's Rune then they're the one who gets the fragment.

I've done 1of the antiquated ruins and I got the calcified fragment but not the card

I really don't know what to tell you. Maybe there's a boss you haven't beat yet that you need for it? Or just beat it a few times and see if that's it. The card is unlocked for me and just tells me how many completions and vip kills I have. Doesn't have a checklist or anything.

I have 41 fragments and I beat antiquated 3 times

Have you beat all the lower level bosses too? Because it sounds like you should have it. Also, is this all on the same character? The fragments are not shared like the ghosts. They're like the Golden Chests, each needs to find them.
