Exotic Sword Quest BUG and ERROR

Finished exotic sword quest, killed warden and big boss within 30 seconds as required, only received credit for killing warden.... not the boss.. Alak-hul or whatever.. QUEST INCOMPLETE.....what??

    BTW, there was no trading of swords between characters... this was a straight up 3 man fireteam running the last part of A sword reforged mission. Both bosses were killed within 10 seconds of each other, first the warden, then Alak-hul... QUEST INCOMPLETE
