The codes distributed for CoD:AW preorders, were not Bungie codes, they were console codes.
If you have a 25 digit code, it is an Xbox code.
A 12 digit code, is for Playstation.

Code distribution was the responsibility of the retailer. Unfortunately, you will have to work with the retailer for any issues with your preorder code.

I love that you're wearing your Blacksmith shader while responding about the Blacksmith shader. It does look pretty badass.

Microsoft has pushed the blame to Activision and it appears that my only portal to Activision is Bungie. That is the reason for this post and I'm hopeful Bungie can "make it happen."

GameStop is. Only they have the record of your transaction. I'm not sure they can do anything at this point, since it has been nearly a year, but if anyone can help with this, it would have to be GameStop.

And that's what led to this post. GameStop sent me to Microsoft. Microsoft sent me to Activision and I can't seem to get in touch with Activision so I'm left with Bungie. Awful lot of work for something as trivial as a shader but in the end, I'd still like it and really don't see any complications from Bungies side.
