You answer your own statement in your post. Not all guns have 100% stability. In real life I would assume the first shot would be more accurate then rapid fire on semi-auto.

In real life your bullets don't go in random directions if your sights are dead on.

Scouts seem pretty damn accurate to me. User error maybe

It's not user error. You can aim a scout rifle right at their head and not get headshot damage if you're shooting too fast. I had a shadow nx01 or whatever it was called and decided to roll it to full auto and try it out. After the initial shot everything counted as a bodyshot.
