I went through the list quickly and didn't see anyone having this...
Ogre and Hydra kills not accumulating toward grim score.
I've tried patrol and story neither give you the kills toward score!?

The fastest way to get ogre kills is on the Shrine of Oryx mission on the moon. Kill the ogre, jump of the ledge, and repeat.

I killed a red health ogre on Moon patrol last night and it was added to the grimoire card (it was the one somniloquism pointed out). Most enemies killed during raids and Prison of Elders also are not counted by some grimoire cards.

Yellow enemies don't count for some reason. You can test if this was the issue by going to he black gate opening on Mars and kill the Hydra there and / or go to the temple of Crota entrance on the moon and killing the ogre there. (The we've woken the hive entrance). Both on patrol and both are normal red enemies.
