You did unlock the Grimoire Card which you receive for "the one ghost on the rock in the ogre room". The Card was Ghost Fragment: The Last Word 3.
The other Ghost you described is Ghost fragment: Darkness 3.
This Ghost is currently bugged, and Bungie is aware of it, as stated in the original Post.
