Is that the one you get from the ghost near the endless steps where you kill a gatelord? If so I did discover that even though I had already got that one when I showed my mate where it was while we were doing the eye if a gatelord mission I did discover it was still there for me to pick up again. Although it didn't show up as rewarding me with anything I do have that grimoire card.
