1) Absolutely sure of it. I check every now and again when I'm passing through. Ghost is gone. Only Ghost I'm missing on Earth. :(
2) As I recall, there was a popup. But I can't remember any longer for sure.
3) I actually picked it up during the last week of the Beta (forgot what month that was. June?) so let's just say Day One/September.
