I watched a dude get his hair caught in one. We'd all warned him to tie it up... -blam!-ed him up royally and left him unemployed.

Wow.... It seems particularly bad with people new to machines, I guess they don't realize how dangerous T-handles can be. I've also heard a lot of stories of people wearing gloves while working on lathes *facepalm*
