I've messed up a few times in a friendship with someone. Let my emotions get the better of me. But I really care about her, and she has a good influence on me.
I feel absolutely terrible about what I did. It eats me alive. I can't sleep, I can't eat. I spontaneously start crying. This person is so important to me.
She's very adamant about her rule of "three chances". How can I convince her that I'm worth making an exception for? I've done so much for her and I expect to do so much more for her.

Ignore her for a while. Either she'll realize she needs you or whatever, or you two will drift apart and stop talking completely. Since she clearly doesn't value you as a friend, it's a win-win situation for you.
