The Elder Scrolls V: Skyrim is rated M, so it doesn't take a lot to earn that rating. Quite the opposite, actually; it takes a lot to earn a T rating when trying to create a realistic, immersive world with a dark side.
Just to note: there are a lot of companies out there that do aim toward a specific rating when developing a game. It's not usually up to the developers or designers, but the head honchos and sometimes even the publisher.
