When you say the last step, are you referring to the return to Phobos to kill the yellow health wizard mission? Because that is the final step, the part before that involves acquiring at least 45 Calcified Fragments before this.

I already got the cf's and did the undying mind mission and killed the ultra

So you got 45 fragments AND completed the special Undying Mind Strike and you're positive all the fragments found are on the same character you have the quest with? Because after doing said strike you should report to Eris who'll give you a Stolen Rune then her quest will update once you hit at least 45. I've done this quest plus sites like this: http://planetdestiny.com/touch-of-malice-quest-guide/ are reporting the same thing.
