You're wrong on his fireteam, from what i have gathered, 3 went into the vault, one came out. Kabr took two hunters (Praedyth and Pahanin) into the vault with him to scout (the vangaurd used teams of 3 at this time still) Praedyth was ripped through time, and Kabr began to go insane (we know his story from here) Pahanin, on the other hand, escaped from the vault to only be murdered by Dredgen Yor later on.
