First time posting in here, but after spending a few hours flipping through my Grimoire I found the "Not Unlocked" cards I was missing.
Some of these I know I've done and they are not unlocked.
[b]Mormu, Xol Spawn [/b]- 3rd round Hive Major spawn in the Phogoth Strike.
[b]Wanted: Twisted Claw [/b]- Found in the underground prowling in the Patrol Moon.
[b]Siege Dancers [/b]- Cerberes Vae III strike, done countless times.
[b]Doubles[/b] - Already posted [b][/b]

The exalted hive you're missing is Urrox, Flame Prince who can be found in PoE as one of the final bosses in level 34 The wanted enemy you're missing is Queen breakers vandal. This is where you can find them. http://youtu.be/s6bQWjvALLE The siege dancers are found in the PS exclusive strike Dust Palace (until September 15) And doubles like you said is bugged.
