I used fist of havoc and tried to hit the bottom of the PoE chest room it. When I hit the bottom I heard "Guardian down." It turns out that I had smashed my friend into the ground since he was right underneath me as we were descending. He never did get that chest since we couldn't revive him. He was just above the hole to the chest room.
