Wasn't a "misadventure" because I didn't die, but I was in the airlock before round 4 of POE and the trap door opened and I fell into the vault below. I was stuck down there for a minute, but then I was teleported back up. There were cabal, but they were just standing there not shooting.

That seems pretty common.

Same happened to me once. All the doors were open in the airlock somewhere between round 2 and 3. I jumped down to have a look at the chests. There were none. That room is really depressing without chests.
