She washed out during the augmentations procedure. Her body rejected the augmentations and ONI rehabilitated her. Her rehabilitation lead the other Spartan-II's to think she had died.

Joy. Yet another bullshit ONI coverup. Unbelievable. The head of the organization is a flipping coverup.

Let me clarify, the lack of her presence after augmentations lead the other Spartans to think she had died. A cover-up nonetheless, but pretty standard for the SPARTAN-II program.

You must admit though, it is pretty ridiculous that the organization responsible for all these coverups is headed by someone who is one of the organization's coverups.
