How you get flaming heads for your character?
Some told me "it just appears", others tell me it's Nightfall. Any real clue/answer to the question?

Complete the Nightfall. It gives you a perk that increases all of your reputation gains, and all of this is signified by your head slightly being engulfed by light blue flames
