I notice it most frequently in Onslaught but is affecting pretty much everywhere but at a lesser rate. The best way i can explain this is you will be at the 1-5 stacks required to proc - Healing Initiative - Upon using your class ability - The Healing Aura Pulse Grenade does not spawn. It commonly fails to activate when walking through a portal - IE like in onslaught and if you use your class ability while on the other side - When you teleport back through the portal - The healing initiative will fail to activate until you walk through another portal or die. I will try and get video evidence of this - But my wife is having the same problem with her warlock there are times her Healing Initiative will fail to activate while at the 1-5 stacks of initiative required to proc Healing Initiative.
