Playing on Xbox Series X. Went through my vault after the Monument update to put all my Crafted Weapons to Tier 5. Most upgraded fine to their respective Tier based on level, but some of the weapons did not actually upgrade Tiers after the Tier upgrade interaction. Tried applying Tiers in multiple locations, both in vault / in Character screen with no luck. I also tried removing weapon mods / legendary ornaments from those that had them, but result did not change.
