Altar of Reflection Catalyst - Witch's Eye
When the Witch's Eye keeper of memory spawns in when doing the mission Altar of reflection: catalyst, it does not open and stays in immune phase. I am only able to kill adds and the knight and his ghost. Is there a mechanic I'm missing to get it to exit immune phase or is the quest bugged?

I made a video of me clearing the boss room. The first time, there were two Hive guardians (1st Knight, 2nd Witch). All the other times, just the Knight. Boss never wakes up.
