I ran into a bug and was able to replicate it by getting a bright engram at the same time that my character dies, happens mostly in crucible but I was able to do it on patrol as well. Bug: The yellow symbol that appears and says Bright engram earned stays on screen, but goes away when returning to orbit. Platform: Xbox One
